Our Patient Portal is our on-line tool for Patient Registration. This lets our patients provide their billing and medical history information prior to their appointments.
How It Works
Faster, Smoother Flow for New Patient Check-in
Online patient registration allows patients to provide their full medical history, demographic, and insurance information at all hours of the day – when it is convenient for them. They no longer come to the office early to fill out paperwork for their first appointment. Instead, when they arrive, they sign in, sit down and are triaged according to their appointment time.
More accurate information
Online patient registration gives our staff more accurate, legible, printed patient demographic and medical history as soon as patients are done online. Information can be confidentially emailed to our office, it will be printed and placed in a chart prior to the patient’s arrival.
Patients can also print out the forms below, fill them out and bring them to their appointment.
Patient Contacted
Patient will be contacted by our office to discuss availability based on the type of appointment requested. We will make every effort to accommodate the schedule convenient for both patient and practitioner given the reason for the patient’s visit.
Saves our patients time
Patients no longer need to take extra time off to arrive at our office just to fill out paperwork, nor do they need to feel rushed in the waiting room. With this feature the information provided via our website is ready and available at your appointment time.
